When arriving for your first appointment you need to have completed an information sheet and medical history. You may do this on this website using our patient portal (link to portal), or upon arrival at the office. If you plan to do this when you arrive, you will need to arrive 15-20 minutes early. It is very important that you have the correct information. This includes:
- Insurance Cards
- Driver’s License or Photo ID
- Military ID card (if applicable)
- A list of your Medications with Dosages and Allergies
- X-ray reports and images or other test results regarding the current condition
- Past Medical and Surgical History
- Guardianship Consent to Treat Patient (if applicable)
