Why do you take my photo and a copy of my driver’s license?

We take your photo and scan your driver’s license into your medical record as a means of identification for our physicians and staff, as well as to meet criteria to protect your identity.  Additionally, this provides us with a copy of your signature and is very helpful for phone calls, questions, and as a visual reference for your medical record.
