KCBJ participates in most HMO-PPO plans including BCBS, Humana, Aetna, UHC and Coventry. We also accept assignment on Medicare patients. Patients can check with their insurance company to verify our participation or feel free to contact the office with any questions. Patients who participate in any insurance plan need to be aware of the policies regarding referrals, precertification requirements, copay, deductibles, services covered, etc. Copays must be paid at the time services are rendered. Primary and secondary health insurance forms will be completed at no charge. Please review our Current List of Insurances with which we participate.

We require that each patient or their legal representative read and sign our HIPAA Privacy Policy and sign this acknowledgement form to file your insurance claims for you.  You may bring this into the office with you at the time of your appointment, or sign it when you arrive.
