How do I get a copy of my medical records? To obtain a copy of your medical records for another physician's continuing care or for your personal file, you will need to complete a written authorization for release of you health information. Copies of records to another physician for continuity of your care will be provided at no charge. All other requests are processed by Midwest Copy Service in our office weekly. They will contact you regarding the fee for that charge. |
