Why do you need my child’s Social Security Number when I am responsible for payment?

When any patient is seen by one of our providers, we need the patient's information to file claims with the insurance carrier.  If you are the Guarantor responsible for payment or the legal guardian responsible for the patient’s medical care, please bring this information with to the appointment for billing and medical record purposes.
