When arriving for your first appointment you need to have completed an information sheet and medical history.  You may do this on this website using your created log-in, or upon arrival at the office.   If you plan to do this when you arrive, you will need to arrive 15-20 minutes early.  It is very important that you bring with you the correct information.  This includes:

• Insurance Cards
• Driver's License or Photo ID
Insurance Company Name and Plan Numbers
Claim Mailing Address
Military ID card (if applicable)
• A list of your Medications with Dosages and Allergies
• X-ray reports and images from Referring Doctor
• Past Medical and Surgical History
• Guardianship Consent to Treat Patient (if applicable)
