How do I get a copy of my medical records? To obtain a copy of your medical records for another physician's continuing care or for your personal file, patient's will need to complete a written authorization for release of your protected health information. Copies of records to another physician for continuity of your care will be provided at no charge. All other requests are processed by DataFile Technologies. They will contact you regarding fees for copies requested. |
