What do I need to provide for Guardian Consent to treat of a minor or incapacitated person? If you are appointed as a Guardian or Legal Representative for the a patient, we will need your authorization to evaluate and treat that person as well as assistance in completing the forms required for our medical record. We also need to obtain a photocopy of the patient’s Insurance ID card(s) and documentation of your Guardianship or legal authorization. If the patient’s insurance plan requires a referral from the primary care physician we will need you to provide for each appointment (including X-rays). Our Physicians and Staff are committed to providing our patients the best musculoskeletal care, and in an efficient and timely manner. If you have other information regarding this patient’s past medical history, or current care issues that we should be aware of, please include that for our medical record. |
